Job Description:

The Medical Assistant will help with Post-Discharge Planning for our patients leaving the Skilled Nursing Facility.  They will assist our Physicians and Nurse Practitioners with contacting other medical practices, scheduling patient in-home visits and other administrative functions.  In addition to the work location of a skilled nursing facility, the position requires travel to complete in-home check ups with the patient and the use of TeleMedicine to communicate with the Physician/Nurse Practitioner.
