As a pioneer in post-acute care, MPAC’s is continuing to break new ground with the introduction of our Transitional Care Program focused exclusively on preventing readmissions within the ultra-critical first 30 days following a discharge from a hospital or SNF. By providing early intervention and education, specialized care, and eliminating variables that often result in readmissions, the transitional care team is well poised to end the frequent flier cycle.
As the heart of the Transitional Care Team the Licensed Practical Nurse is the primary point of contact tasked with helping to manage the patient's medical care. The License Practical Nurse will help schedule the patient's first at-home visit, follow up with basic on-site care, make sure they are following the post-discharge plan and help facilitate our TeleMed system for the patient to speak directly to the Nurse Practitioner.
At MPAC, we’re looking for License Practical Nurses with strong interpersonal skills. In addition to scheduling the medical care for patients, the individual must be able to build trust and rapport with facility personnel, physicians, APN’s, patients and caregivers.
Helping coordinate/schedule home visits
Assisting with in-home patient visits
Basic Knowledge of taking vitals signs
Assisting in the TeleMed process of the practitioner/patient communication
Ability to work in the U.S.
A great attitude with a desire to deliver the best patient care
Tech Savy (assisting with TeleMed)
Car & Driver's License
